Twin studies compare the rates of ADHD in identical twins, who share all their genetic material, with fraternal twins, who share around half. The higher concordance of ADHD in identical twins compared to fraternal twins provides evidence for genetic contribution. No single gene causes ADHD. Like most common neurodevelopmental conditions, ADHD is polygenic: many common genetic variants each contribute a small amount to ADHD risk. The combined effect of many such variants, known as polygenic risk, accumulates to produce elevated overall risk.

If a sibling has ADHD, the risk for other siblings is elevated compared to the general population. These family patterns reflect both the genetic contribution and the possibility of shared genetic variants across generations.
Some ADHD cases involve de novo mutations: genetic changes that arise newly in the affected person rather than being inherited from a parent. De novo mutations associated with ADHD are more commonly found in individuals with more severe ADHD presentations and those with co-occurring intellectual disability.
Environmental factors can contribute to ADHD risk, particularly when they interact with genetic vulnerability. Prenatal exposure to nicotine or alcohol, premature birth, very low birth weight, significant early childhood adversity and prenatal exposure to certain toxins have all been associated with modestly increased ADHD risk in epidemiological studies. These factors do not cause ADHD on their own in most cases: most children exposed to these risk factors do not develop ADHD.
Despite high heritability estimates, genetics does not explain everything about ADHD. The same genetic variant does not always produce ADHD, and the expression of ADHD varies considerably even within families. ADHD is not caused by vaccines, diet, screen time, poor parenting or any other environmental factor that has been thoroughly studied. Parents should not feel responsible for their child’s ADHD.
