Is Autism Genetic?

Research strongly indicates that autism has a significant genetic component. Twin studies consistently show high heritability: studies have estimated autism heritability at between 64 and 91 percent, meaning genetics explains a large proportion of the variation in autism risk. However, autism genetics is complex. No single gene causes autism in most cases: hundreds of genes have been identified that each contribute a small amount of increased risk. Many autism-associated genetic changes are de novo, meaning they arise newly in the child rather than being directly inherited from parents.

What twin studies show

Twin studies are a primary method for estimating genetic contribution to a condition. When one identical twin is autistic, the other is autistic in a significantly higher proportion of cases than would be expected by chance. Studies of autism in twins consistently show high heritability estimates. A large study published in JAMA Psychiatry estimated autism heritability at around 83 percent.

Hundreds of genes, not one

Unlike single-gene disorders, autism is polygenic: many genes each contribute a small amount of increased risk. More than 100 genes have been identified with strong evidence for involvement in autism risk and hundreds more have been implicated with varying degrees of evidence. Large-scale genomic studies continue to identify new genetic variants associated with autism.

De novo mutations

A significant proportion of autism-associated genetic variants are de novo: they arise as new mutations in the child rather than being inherited from either parent. De novo variants are found more commonly in people with autism and intellectual disability than in those with autism alone.

Inherited risk

Not all autism genetics involves de novo mutations. Common genetic variants, each conferring small amounts of increased risk, can be inherited from parents. This polygenic risk is distributed across the population and partly explains why autism tends to run in families even without a single identified gene. Parents of autistic children have modestly elevated rates of autistic traits themselves.

Genetic syndromes and autism

Some genetic syndromes including Fragile X syndrome, tuberous sclerosis and Rett syndrome are associated with high rates of autism. For people with these syndromes, the genetic cause of the autism is more clearly identified. These syndromic forms of autism account for a minority of overall autism cases.

What genetics does not explain

Even with high heritability estimates, genetics does not explain everything. Identical twins are not always both autistic despite sharing all their DNA, suggesting that non-genetic factors also play a role. Prenatal environmental factors have been associated with modestly increased autism risk in some studies. Vaccines have been exhaustively studied and are not associated with autism: this claim was based on a fraudulent and thoroughly discredited study.
