What Is ADHD?

Attention Deficit Hyperactivity Disorder (ADHD) is a neurodevelopmental condition characterised by persistent patterns of inattention, hyperactivity and impulsivity that interfere with functioning and development. The NHS estimates that ADHD affects approximately 1 in 20 school-age children in the UK, and it is now widely recognised that ADHD continues into adulthood in the majority of cases: millions of adults in the UK live with ADHD, many of whom were not diagnosed in childhood. ADHD is not caused by bad parenting, poor diet or laziness: it is a neurodevelopmental condition with a strong genetic basis and identifiable differences in how the brain develops and functions.

ADHD is characterised by difficulties in three main areas: attention regulation, impulse control and activity level. People with ADHD may have difficulty sustaining attention on tasks that are not immediately engaging, act before thinking, interrupt frequently, have difficulty sitting still and find it hard to organise their time, prioritise tasks or follow through on plans. These difficulties are persistent, present across multiple settings and cause genuine functional impairment rather than being within the typical range of variation in these traits.

ADHD is categorised into three presentations in DSM-5: predominantly inattentive, predominantly hyperactive-impulsive and combined. The combined presentation, where both significant inattention and significant hyperactivity-impulsivity are present, is the most commonly diagnosed. The predominantly inattentive presentation, sometimes still referred to colloquially as ADD, does not involve significant hyperactivity. The predominantly hyperactive-impulsive presentation involves significant hyperactivity and impulsivity without the inattentive component being as prominent. Understanding which presentation is present helps guide support and treatment decisions.

ADHD is not a character flaw: One of the most persistent and damaging misconceptions about ADHD is that it reflects a lack of effort, poor willpower or inadequate parenting. ADHD is a neurodevelopmental condition with a strong genetic basis and identifiable differences in brain development and function, including in dopamine and norepinephrine signalling systems. People with ADHD are not choosing to be inattentive or impulsive: they have a neurodevelopmental difference that makes attention regulation and impulse control genuinely harder than for neurotypical peers.

ADHD across the lifespan: ADHD was historically viewed as a childhood condition that children grew out of. Research has established that this is not the case for the majority: ADHD continues into adulthood in around 60 to 70 percent of those diagnosed in childhood. The way ADHD presents may change as people develop: hyperactivity often becomes less visible in adulthood, while inattention, disorganisation and emotional dysregulation may become more prominent challenges.

ADHD affects several areas of development and daily life. The features below are core to ADHD, though they present differently between individuals and across different life stages.

Attention regulation difficulties

People with ADHD often have significant difficulty sustaining attention on tasks that are not intrinsically interesting or immediately rewarding. This is not a global inability to pay attention: people with ADHD can often focus intensely on activities they find engaging, a phenomenon known as hyperfocus. The difficulty is in directing and sustaining attention according to external demands rather than internal interest, which creates significant challenges in school, work and daily life management.

Impulsivity and inhibition difficulties

Impulsivity in ADHD refers to acting before thinking, difficulty waiting, interrupting others, making decisions without considering consequences and difficulty delaying gratification. Impulsivity affects social relationships, safety and decision-making. It reflects a difficulty with response inhibition, the ability to pause and consider before acting, rather than a deliberate choice to behave impulsively. Impulsivity can cause significant problems in social and professional settings.

Hyperactivity

Hyperactivity in ADHD refers to excessive movement, restlessness, difficulty remaining seated when required, talking excessively and a general sense of being driven by a motor. In children, hyperactivity is often visibly apparent. In adults, hyperactivity may present more as internal restlessness, a feeling of always needing to be doing something, difficulty relaxing and rapid task-switching. The presentation of hyperactivity typically becomes less overtly physical as people age.

Executive function difficulties

Executive functions are the cognitive skills that enable goal-directed behaviour, including planning, organisation, time management, working memory, cognitive flexibility and self-monitoring. ADHD has a significant impact on executive function, which is why people with ADHD often struggle with tasks that require planning ahead, managing multiple demands simultaneously, remembering to do things and switching effectively between tasks. Executive function difficulties affect academic achievement, employment and daily life management.

Emotional dysregulation

Emotional dysregulation, difficulty managing the intensity and duration of emotional responses, is recognised as a significant and often overlooked feature of ADHD. People with ADHD often experience emotions more intensely than neurotypical peers and may have difficulty returning to a calm state after emotional arousal. Rejection sensitive dysphoria, an intense emotional response to perceived rejection or criticism, is commonly reported by adults with ADHD and can significantly affect relationships and wellbeing.

Co-occurring conditions

ADHD frequently co-occurs with other neurodevelopmental and mental health conditions. Anxiety disorders affect a large proportion of people with ADHD. Depression, autism spectrum disorder, dyslexia, dyspraxia, tic disorders and sleep disorders are all more common in people with ADHD than in the general population. Co-occurring conditions significantly affect the support and treatment needs of people with ADHD and should be assessed and addressed alongside the core ADHD diagnosis.
